Pay Range: $33 - $68/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Serve as the key liaison for Alzheimer’s patients and their families
• Coordinate care across pre-op, acute, and post-care phases
• Educate, empower, and advocate for patients using evidence-based best practices
• Collaborate with a multidisciplinary team to enhance patient outcomes and care experience
• Lead performance improvement initiatives that make an impact

Key Requirements and Technology Experience:

• Skills- BLS, experience as a Registered Nurse.
• BSN or MSN (required)
• Active RN License (Compact or GA) + BLS Certification
• 3+ years of adult acute care experience (bariatric, spine, or case management experience a plus)
• Strong communication, leadership, and organizational skills
