Job Title: Visiting Nurse (RN) - Floating Territories

Company: Home Health Care, Inc.

Schedule: Full-Time

Annualized Salary: $75,000 - $110,000 (paid per visit)

As a Visit Nurse, you will build meaningful 1-on-1 relationships, see your patients' progress and hit their goals, and stay happy and healthy in the comfort of their own home! All of our clinicians work in strategic "territories" to see clients (approx. 6 to 7 per day), and your goal in this position is to help out with patient load fluctuations across multiple territories.

Registered Nurse - RN Visit Nurse Responsibilities:

• Assessing and re-evaluating the client’s condition and needs as required.
  
• Provide patient cares according to client’s care plan and needs.
  
• Teach the client and/or family members about cares and support skills.
  
• Communicate regularly with RN Case Managers, Supervisors, and the Care Team.
  

Registered Nurse - RN Visit Nurse Qualifications:

• Possess a current Registered Nurse license in the State of Minnesota.
  
• Trained in skills necessary to provide nursing care to a wide variety of medical conditions.
  
• Demonstrate basic computer knowledge and ability with an aptitude to learn company applications.
